How to become a long distance patient?

This service is offered to:

  • Couples that live more than 4 hours of driving outside of the Montreal area.
  • Couples that would like to have their medical file reviewed before commencing treatment at the McGill Reproductive Centre.
  • Couples that would like to get a second opinion.

How to get started? 

1. Download and Print the check list of tests and documents that need to be completed before we can open you a file. If any documents and/or tests are missing your file will not be processed Out- of- town patient check list

2. Print, complete and send us a copy of the out-of-town patient consent form (non-canadian residents)

3Print, complete and send us a copy of the self-assessment form

4. Send us a copy of your medical file giving details of all previous infertility tests and treatments.

 5. The following tests are ideally performed on the third day of the menstrual cycle. If day three falls on a weekend, the tests can be done between day two to day five of the menstrual cycle.

    • Blood test: follicle stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2).
    • Transvaginal ultrasound scan: The ultrasonographer needs to determine if the ovaries are polycystic, count the number of follicles in each ovary and record the average diameter of each of the follicles.
    • Please bring a copy of the requisition to your ultrasound appointment and give it to the sonographer to use as a guide.
    • Please send all tests at the same time. Sending in parts may delay file opening.

6.Fees Payable: 

    • Non-Canadian Resident: refer to our finance section for the fees.
    • Canadian Resident: refer to our finance section for the fees.

In Person Medical consultation fees are payable only in cash
Phone consultation fees are payable only with a credit card

7. Once a copy of your file has been received and your payment has been processed it will take approximately 2 -3 weeks to have your medical file reviewed by a doctor.

8. Once your file has been reviewed a phone consultation will be arranged at a designated time.

9. Once a treatment plan has been determined your file will be assigned to an IVF nurse. Since every treatment is specific to each individual person a nurse will contact you to give you details specific to your file.

10. No Treatment plan will start prior to completion of all of the above.

For any additional telephone consultations, a fee of $150.00 for Canadian residents and $200.00 non-Canadian residents is applied.
